Forms must communicate a large amount of instruction and response. MVA forms are one of the worst forms people consistently encounter. In this redesign, I was aiming to achieve clarity, mobile accessibility, and better user experience. The original form was packed onto six printed pages. The redesign spreads this large amount of information onto 55 pages in a mobile app. By having more pages, users have much less information on each page. Thus, the users will be less stressed and will fill in the form at their own paces.
